Their South Tyneside Social Prescribing teams can help to:
- chat to you about your well-being
- help to build your knowledge, motivation and confidence to make changes
- connect you to activities, groups and opportunities in your area
- develop ways you can manage your mental and / or physical health conditions
- Support you to navigate your care, if multiple services are involved
Who can access the service
Anyone aged 16 or over living in South Tyneside can access services free-of-charge.
