Overview

The common purpose of First Contact Clinical is to enable healthy change in the people and places it is needed most.

They provide education, training and community based services which support "healthy change" in people and professionals living and working in disadvantaged areas and communities.

They specialise in delivering behaviour change skills training. Their team of trainers apply their knowledge and experience of holding person centred, motivational conversations while working with people in the community.

They also run a South Tyneside Social Prescribing Service to make a difference to the lives of the people they work with, through promoting the knowledge, skills and confidence needed to practice self-care. Appointments are longer than typical GP appointments. This gives individuals time to talk about what matters to them.

  • Identify what is affecting well-being and how people would like things to be different
  • Build knowledge, motivation and confidence to make changes
  • Set and achieve personal goals to improve well-being
  • Connect with people, activities, groups and opportunities in their area
  • Develop strategies for managing mental and/or physical health conditions
  • Support navigating/coordinating care, if multiple services are involved

 


Who can access the service

Anyone aged 16 or over living in South Tyneside can access services free-of-charge.
